Collect Customer Feedback

Gather open-ended feedback with video, audio, and text responses.

A feedback form lets customers tell you what they think in their own words - by typing, recording audio, or recording video. This guide walks you through building one.

What you'll build

A feedback form with:

  • A welcome screen with a personal video
  • A rating or choice question
  • An open-ended follow-up for detailed feedback
  • An optional Contact node
  • A thank-you end screen

1. Create the form

From your dashboard, click Create Form and name it (e.g. "How are we doing?" or "Share your feedback").

2. Add a welcome screen

Drag a Button node onto the canvas. Write a short message asking for honest feedback - something like "We'd love to hear what you think. It only takes a minute." Set the button text to "Start".

Record a video of yourself (or a team member) asking for feedback. A personal ask goes a long way - people are more likely to give thoughtful responses when they feel someone is genuinely listening.

3. Add a rating question

Drag a Choice node and frame it as a satisfaction question:

  • "How would you rate your experience?" with options like: Amazing, Good, OK, Could be better, Poor

This gives you a quick quantitative signal you can track in Results.

4. Add an open-ended follow-up

Drag an Open-Ended node and ask something like "What could we do better?" or "Tell us more about your experience."

Leave all three response types enabled (text, audio, video). Some people prefer typing, others find it easier to just talk. Giving them the choice means you'll get more honest, detailed responses.

5. Add a Contact node (optional)

If you want to follow up on feedback, add a Contact node with Email and First name. Make it optional so anonymous feedback is still possible - some of the most honest responses come from people who'd rather stay unnamed.

6. Set up the end screen

Thank the respondent: "Thanks for your feedback! It really helps us improve." You don't need a call to action here - keeping it simple shows you respect their time.

7. Preview and go live

Walk through the form in Preview mode. Check that video plays, the open-ended node accepts all three response types, and the flow feels natural. Toggle to Live and share it.

Tips

  • Keep it to 3-4 screens - one rating, one open-ended, optional contact, end screen. Feedback forms should be fast.
  • Let respondents choose their format - text, audio, and video each capture different kinds of insight
  • Don't require contact details - anonymous feedback is often more candid
  • Check Results regularly - the Choice node gives you a quick pulse, and open-ended responses show the detail behind the numbers
