Create a Quiz

Build a scored quiz with instant feedback and results.

Quizzes are one of the most popular form types. Respondents answer questions, each answer earns points, and the end screen shows their score. This guide walks you through building one.

What you'll build

A multi-question quiz with:

  • Scored Choice nodes (correct answers earn points)
  • Instant right/wrong feedback after each question
  • An end screen that displays the final score

1. Create a new form

From your dashboard, click Create Form and give it a name (e.g. "Product Knowledge Quiz").

2. Enable scoring

Open the form settings by clicking the gear icon in the top toolbar. Turn on Scoring - this tells Clipform to track points across the form. You can also turn off Back navigation so respondents can't go back and change their answers, and turn on the Step counter so they can see how far through the quiz they are.

3. Add your first question

Drag a Choice node onto the canvas. Write your question text, then add answer options. To mark the correct answer:

  1. Click on the Choice node to open the settings panel
  2. Toggle Answer key on
  3. Click the checkmark next to the correct option
  4. Set the Score for the correct answer (e.g. 1 point)

You can assign different point values to different questions. A harder question might be worth 3 points while an easy one is worth 1.

4. Add feedback

In the same Choice node settings, turn on Show feedback. After a respondent selects an answer, they'll see a brief correct/incorrect indicator before moving to the next question.

5. Add more questions

Repeat steps 3 and 4 for each question in your quiz. A typical quiz has 5 to 10 questions. Each Choice node connects to the next automatically.

6. Set up the end screen

Click on the End Screen node and configure it:

  • Turn on Show score to display the respondent's total score
  • Write a congratulations or thank-you message
  • Optionally add a Call to action button to restart the quiz or visit a link

7. Add media

What makes a Clipform quiz different from a static quiz is video. For each question, record or upload a short video clip to play alongside the question text. This turns a standard quiz into a conversational experience - like a host asking the questions directly.

8. Preview and go live

Click Preview to walk through the quiz yourself. Check that scoring, feedback, and the end screen all work as expected. When you're ready, toggle the form to Live and share it.

Tips

  • Keep questions focused - one concept per question, 3 to 5 answer options
  • Mix in easy wins - start with a couple of straightforward questions to build confidence
  • Use video for narration - record yourself reading the question or giving a hint. It's more engaging than text alone.
  • Check your scores - preview the form and verify the end screen shows the correct total
